At Carisbrooke Dental Practice we take your privacy extremely seriously.
This policy explains what information we might gather from you whilst you browse our website and what we do with information that you send to us, or information that your dentist might send to us as part of a referral.
Information gathered by our website
Like many websites we use Google Analytics to analyse the use of our website. Things such as the pages you visit, what date and time, how long you spend on a webpage, and technical information such as your IP address, web browser type and the type of device you are using. This information is used to analyse which aspects of our website are working well, and which areas need developing.
This information is gathered through the use of a browser cookie (visit https://ico.org.uk/for-the-public/online/cookies/to learn more about cookies) that helps us to track your usage of our website.
Our website will prompt you upon your first visit to our site as to whether you are happy to accept this cookie or not. By default, our website is unable to track your visit until you consent to this cookie. Figure 1 below illustrates how this looks on our website.
Information supplied to us by you
At times you might supply information to us via our website contact form, by sending us an email, or perhaps by sending us a message via our social media channels such as Facebook or Twitter.
The information we gather via these channels will be used for the purposes of your request (e.g. requesting the availability of an appointment) and may be shared internally with members of the Carisbrooke Dental Practice staff. In certain cases, and only where necessary to fulfil your request, the information may be shared with third parties, such as Dental Laboratories who might be responsible for creating your dentures, crowns etc.
If you provide us with contact details such as your telephone number or email address, we will use this for the purposes of contacting you in relation to your request.
Information supplied to us on your behalf
If you are a patient of another dentist/dental practice and need to be referred to us for specialist treatment, your dentist will complete a referral form by either completing an online form on our website, emailing us an electronic referral document, or by sending us the referral information using a paper form or letter.
We will only use this information for the purpose of the referral, however we may contact you directly to confirm specific information to ensure that you receive the right advice and care.
Information held by us whilst you are a patient of the practice
To provide you with a high standard of dental care and attention we need to hold some personal information about our patients. This personal data includes all relevant past and current medical and dental conditions, and information such as your age, NHS number address, telephone number, your GP etc.
We need to keep comprehensive and accurate information about you to ensure that you are provided with safe and appropriate dental care. We also use mobile phone numbers to send out reminders of appointments as a courtesy.
We will retain your dental records whilst you are a patient of Carisbrooke Dental Practice and after you cease to be a patient for at least 11 years or for child until the age of 25, whichever is the longer.
Security of your information
Personal information about you is held in the practice’s computer system and/or in a manual filing system. The information is not accessible to the public and only authorised members of staff have access to it.
Referring Dental Practices
Information regarding your patients will be sent by letter or email. Emails with private information regarding the patient will be password protected. The passwords will be phoned or texted to you.
Disclosure of Information
We may need to disclose personal information about you to other Health Care providers or Public Sector Agencies (for example your NHS dentist) to ensure that you are provided with appropriate care or treatments both at Carisbrooke Dental Practice or the other organisations.
Your information may be disclosed in connection with legal, or prospective legal proceedings.
Occasionally your health insurance provider may contact us to clarify claims without co-operation from ourselves they may delay payment to yourselves.
Disclosure only takes place on a “need to know basis” so that your personal information is not shared unless absolutely essential.
We do not disclose your information to Third parties for the purposes of marketing.
How to access the information we hold about you
You have the right to view your personal data. If you contact us we will need signed permission to release your notes to you.
How to update your information
If your personal data changes (for example if you move house, change your phone number etc) please let us know so that we can ensure the information we hold about you is current and accurate.
